A patient was seen for yearly physical but also had three complaints and therefore was a comp history, comp exam, and a mdm of moderate. So I coded 99397 and then to do an office visit I was not sure what to do because of course the exam and history are always high level with a preventative exam. I need to put the history level, Exam and MDM so how do you do that when it also is preventative? Thank you

honestly you need two separate notes so that the ov part is a separate hx exam and mdm from the preventive. With one note it is hard to separate out the issues and what is part of the preventive cannot count toward the ov.
