Can a provider bill for a child well exam as well as a sick visit for medical clearance for dental work under anesthesia?

In the medical clearance he does mention daytime and night time accidents and behavior concerns.

I thought those are elements that are included in a child well exam. Help anyone :/
 
What is the insurance type? since the E& M coding guide lines have changed would it be based on time or what was done? wouldn't be just a history & physical comment? was there treatment for it ? I would say no, but I could be wrong
