1. We had a patient that completed all of the preventative test for a physical but, had to leave and didn't complete the HME visit. How should we code this visit? What level of visit should we tie the preventative test to.

2. The vitals were taken for an acute visit, but the pt left before being seen. How should we code this visit.

3. The patient has a visit with all of the preventative vitals but, we discover that this patient has had a physical elsewhere so our provider didn't complete a physical, what level of visit should we tie the vitals to.
