Hi all... thanks in advance to anyone who can help. This is a 2 part-question...

I am struggling to find in the CPT coding book on how to code appropriately for preventive services when done in the home. My clinic is on its way to becoming a Patient Centered Medical Home and we will be providing services in the home setting. I have found the 99341-99350 codes which represent for sick patients, but I cannot find anything that tells me how to code when performing well care by the physician at the patient's home.

Also, we are a pediatric clinic and will also be seeing newborns in the home (after discharge from the hospital or birthing center) and am struggling to find the codes for that as well. I found 99461 for this but if I am reading the guidelines right, it says prior to home dicharge for this.

Please help!!
