Wondering if anyone has ever run into this issue. One of our practices see patients from two long-term care facilities in the area. Most all of these patients are disabled either as a result of an injury or other medical conditions. One of these facilities requires the patients to have a yearly preventive exam, I know this is administrative, but wouldn't the patient be eligible for a annual well visit anyway, based on the plan? Usually they have Medicare. Currently these are being coded as established patient visits with the patient's chronic conditions as the dx. I am not sure what is correct. I have to have all possible documentation to support any change in the current process, as the doctors can be very resistant to change.
