Hi--We submitted this question during the recent webinar "An Ounce of Prevention" but apparently there wasn't enough time to address it. Maybe someone can help? Here's the scenario:

A patient presents for a Preventative exam. During the exam the patient states that they would like the doctor to look at a lesion. The doctor treats the lesion with a minor procedure. The minor procedure has a global period of 10 days. A problem focused E&M with modifier 25 was billed, the lesion code was billed and a preventative E&M code was billed. Should the Preventative E&M code be billed with a modifier 25 also? Our code review software is denying the preventive code.
