Hi,
If a patient presents to an office for a preventive visit(annual) exam, but then does have a problem which the dr addresses. would it be appropriate to bill a preventive codes (99381-99397) and office visit 99201-99215?
If so where can I find resource with this information-I have looked at the preventive guideline.

It is appropriate to bill both together if it is significant enough to qualify as another encounter, apply modifier 25 to the office/outpatient code to indicate a significant, separately identifiable E/M service was provided. You should be able to find this in your CPT book. If you have the CPT 2009 edition from the AMA, this is found on page 29 under Preventive Medicine Services. Hope this helps! :)
