The patient had a previous hysterectomy and now had a 58720 ( Salpingo-oophorectomy. The insurance is wanting a consent form but the office didn't have the patient sign one because she was previously sterilized. She had this procedure because she had breast cancer and it was recommended to have the procedure done. The diagnosis that we have used was v10.3 personal history of malignant neoplasm of the breast. I was wondering if anyone had this issue and how do you resolve it.
