I just recently have begun to get denials from Anthem regarding cpt 90471 and 90460 stating for denial " in accordance with cpt guidlines, add on codes require the submission of the primary/base code and must ever be reported as a stand-alone code. In both of these instances an office visit was billed with the 25 modifier. Thanks,
