Can a primary care doctor code out 96156 or is it strictly just behavorial health?
 
I wouldn't recommend it. While it requires a physical diagnosis to bill, it is really designed for mental health professionals to help patients cope with or manage a physical health condition. Further, alot of insurance plans have a carve-out for mental health and substance abuse, and those carve-outs usually are not available to anyone outside of a MH/SA specialty.
