Hi,
I work for a practice that just informed me today that they have been a "Primary Care Medical Home" since last October and they would like to know which codes they should be using to be reimbursed properly for this. I've searched all over Google and I can't find anything that states which codes to use but I have found articles that hint to specialized codes and/or modifiers. Any input is appreciated!
