Chief Complaint C4 Tetraplegia and Neurogenic Bladder; First listed assessment diagnosis: Hypertension. The reason for the admission is the C4 Tetraplegia and multiple additional diagnosis. Since provider does not list this as the first diagnosis in the assessment, can I code the C4 Tetraplegia as the primary diagnosis?
