Our palliative program is beginning PCM and as we set up the following question came up. Is a verbal consent enough for this service or is a written consent form needed?
 
Hi,
From all of the information I have pulled together to educate our providers on PCM, the documentation must have indication of verbal consent from the patient. We did take it further though and created a flyer for the patients who have given verbal consent what PCM entails, the possible cost and that they can Opt out at any time. We do not obtain written consent, only verbal.
