A patient comes in for irregular periods. The provider believes it is stress related and does a pap and pelvic exam and Rx Birth control bills. My question is would I code N91.2 for the irregular periods, Z30.011- initial Rx birth control bills, and Z12.4-pap smear? I'm not sure how to code for the pap. I'm used to seeing paps on WWE's only. Please help!
 
Yes the appropriate dx for a pap outside of the annual WWE is Z12.4. That is what I use for repeat paps done at the 6 month mark or when they are being done at an office visit.
 
The PAP was not performed for screening purposes it was done due to the irregular periods. You would not code the screening code at all. when you send the smear you should mark the requisition for the dx of the irregular menses not screening. There is no procedure code for the PAP or pelvic it is the office visit code do not use the Q and the G code that you use for the well woman.
