Hello can any one helps I'm trying to figure out the CPT code and how to bill this on the cma 1500 form. Upper right arm, 1400mg,
Probuphine implant. I know POS and the modifier and the DNC #

Please if any can help I will pay it for my great grandma Who got the implant for pain management doctor refuses to bill it and is charging her. Her insurance company covers the procedure but I need to know how to Bill it so I can bill her insurance company so she can get reimbursed the money she had to pay


Please it would be so appreciated it would a to blessing for my grandma
 
I didn't think you could legally fill out a 1500 form if you are not the provider of services? Id follow up with the payer to see if they have an alternate claim form to attach the itemized invoice to.
