procedure-antegrade ureteroscopy with holmium laser lithotripsy, ureteral stent insertion and nephrostomy tube exchange

nephrostomy tube removed, nephrostomy tube tract was simply dilated and a sheath placed, cystoscope was placed through the sheath into the renal pelvis
attempts at placing a Nephro Max UroMax balloon unsuccessful, holmium laser of impacted stone, basketed and removed, 4.8x26 JJ stent placed and a 20 french nephrostomy tube placed

not even sure where to start with this one, any help with codes much appreciated

I would suggest the following coding for your clinical scenario:
50961 for the antegrade treatment of the ureteral stone,
50553 for the antegrade stent placement, and
50345 for the change of the NT.
