The doctor did a polypectomy on a pt during visit she had cervial polyp, and had other issues. Does that fall under e/m visit, or is there a procedure code for polypectomy. OH IT WAS SENT TO PATH.

Look at CPT Code 57500 and see if documentation describes. Then append a 25 modifier to the E/M code for the additional issues. Removal of polyp is not included in the E/M.
