I code for Interventional Radiology. Our techs usually give contrast, 2% lido, lido w/epi and NS hep bags during a procedure. I recently noticed that the techs were documenting after they left the procedure room. This wouldn't be a problem but they were documenting the meds with the current time which was after the procedure was finished and after the patient left the procedure room. My question is should these types of meds be documented between the procedure start and finish time or is it ok to document from the time the patient is in the procedure room until they are out?

IMO the timeline is corrupted if the time of administration is not noted, whether it is charted as a "late entry" or done in real time. Seems the med could be documented by the click of a key with the time automatically posted.
All that being said, unless the code requires a time frame for reference, such as fluid infusions.
