I worked for a neurologist and we do psychological testing for patient. Do we still need to have additional notes for the testing to be able to charge the patient? We are doing addendum right now for the testing, is that ok?
 
It's best if you have distinct notes of the testing, the name of the test used, and the outcome of the test results in the patient chart.
