My physician fee shedule with a commercial plan follows Medicare's guidelines to determine which codes are considered diagnostic/therapeutic. I have searched CMS's website and cannot find this information anywhere. My FI is CahabaGBA. Can anyone advise where I can find this?

I have conversion rates for the following categories:
Surgery
Radiology
E&M
Diagnostic/therapeutic

Thanks, I would appreciate any guidance
