If a patient has outpatient procedures done in a hospital, then 2 months later has inpatient procedures done would that be considered a subsequent hospital care E & M?
 
No. Each hospital visit is separate. So if the patient was seen outpatient last week and then is admitted today, your doctors would do an Initial Admit E/M (if they are the admitting doctor) and then use the subsequent E/M codes for any following days that they are in the hospital. If they are discharged tomorrow and then re-admitted a couple days later, you start again with the Initial admit, etc.
