This was billed incorrectly initialy by staff. J0885 x 40 units, 96372, J3420 x 1 unit, then 96372 again. Dx 285321, 585.4, 281.1

Medi/Medi: Medicare processed the J3420 which they applied to deductible. And rejected the rest of the claim. They are using denial codes 50, M25

How do I get Medicare to reverse the processing that they did, so that we can resubmit a corrected claim and get paid?
