Does anyone have any specific examples of documenting time/notes for 99354 and 99355 other than what is in CPT 2009 and CPT Changes for 2009?

Here's what I've trained my physicians to record ... after documenting their usual note ... "I spent an additional 35 minutes in direct patient care due to __(fill in the blank)__."
The key for prolonged service codes is that the time has to be over and above that required for the basic service being coded.
