In our podiatry office we keep walking boots on hand and when a patient is in need they are given out accordingly. These HCPCS codes fall under an LCD policy that requires that there must be proof of delivery documented. Is this only a requirement for a "supplier" or is the physician considered the supplier because he is the one giving out the DME versus writting an RX and sending the patient to a medical supply store? The LCD is unclear on what meets the definition of a supplier. Thank you!
 
First does your doctor have a DME with your local DMAC jurisdiction. Then your physician must have both the prescription (which can be documented in the patient's office note) and signed proof of delivery. If your physician is not a DME provider for your local DMAC jurisdiction then you must have a copy of the prescription in the patient medical record and send him/her to a DME supplier.
