Wiki Proper billing for assistant surgeon

I am needing to know if there is an assistant surgeon with the same TIN as surgeon, can it be billed on one claim or does the claim need to be separated? One for surgeon and one for assistant? I looked on medicares billing guidelines and couldn't find anything. I have to have in writing. Any help would be greatly appreciated!

You will need to submit two separate claims with a modifier on the assistant surgeon claim. Keep in mind that only certain CPT codes are payable in the instance.
