Proper OP Report Documentation

We are going back and forth with the doctor's on how to properly document everything that they do on ASC-Out PT OP Reports. I have brought them articles and documentation but they still want to argue that they are documenting correctly. It is lack of documentation that I am fighting with. Does anyone have other websites, articles, anything else that I can show the doctors so that I can get them to document everything that they do.
