Is it okay to code procedures from a signed hard-copy form/worksheet that has handwritten notes by the provider and has check boxes or does it have to be dictated? I am also questioning when the guided ultrasound is performed if the provider can write the findings/ notes on the worksheet or does that provider have to dictate a separate note with specific details? The form will be scanned into the account and the image will be kept on a system in the clinic. Is the form for the biopsy and ultrasound okay to code from and what are the specific note requirements for the ultrasound to be able to bill it?
