What are the exact requirements for coding for prothombin time? Here is an example of information I was provided: "patient has to be discharged within 30 days of DOS, no provider visit same day and documentation has to say the provider asked the pharmacist to see them". I have researched online and cannot find any information that explains so in-depth, other than there cannot be a provider visit the same day. Can anyone please let me know the proper coding requirements for this service? Thank you.
