Does anyone know where I can find information on provider based billing? I need to know what you are allowed and not allowed to bill on the technical side of it. Specifically facility fees when there is also a procedure or a flu shot.
 
My organization has been doing pbb for over 4 years now. We use a scorecard to determine the level of facility E/M. We do not have anything on the scorecard that is separately billable, such as a vaccine admin. If the patient only comes in for a service that has a billable CPT code, we only bill for that service and not the facility E/M.

We used a consulting firm to help us get set up, so I don't have CMS links.
