Wiki Provider treats only certain symptoms out of many related to an underlying condition

For instance, we have a patient where the underlying condition is a concussion without loss of consciousness and the provider is only treating a single symptom - post-traumatic headache - which resolved prior to the most recent encounter. Does the concussion still get coded if the patient is being treated by another provider that isn't in our practice for a different symptom of the same concussion?
 
I'm a little confused by your phrasing, but isn't this where the sequela comes in? So if he's being seen specifically for the headache, you'd code headache first and then the 2ndary code would be the S code for the concussion.
