How do billing rules apply to the following situation...

The patient comes in for a preplanned PRP injection. The patient paid for the PRP.

If I were billing the injection, the E/M visit would not be billed. However, since the patient paid for the PRP, then no injection is being billed. Would it be appropriate to bill the E/M visit or would that be considered double dipping?

Double dipping. A cursory exam is included in any injection regardless if the patient pays for it or not. So unless you have something to qualify for an E/M you can't bill it anyway. Additionally, this was a pre-planned injection so no E/M at the injection visit. The decision for the injection has already been made.
