I was wondering if any psych coders had a good example of a psych initial assessment where the physician was coded 99222 or 99223. This can be either an admit or a consultation. Meaning excellent documentation of an extended HPI, Complete ROS, Complete PFSH, Comprehensive Exam using 1997 guidelines, and MDM of Moderate or High level.
I have searched everywhere on the internet and haven't had any luck. So I was hoping someone could scrub a document for me and cut and past it here. I have a physician asking for an example.
