Wiki Psych Exam prior to Stim Implant

My provider has a paper questionnaire some patients complete for their Psych Exam prior to Neuro Stimulator Implants. The questionnaire is then faxed to a company which scores the test and returns the results. The doctor simply reviews the report. Does anyone bill for the review and if so what code do you use?
 
We do not bill for the review as most insurance do not reimburse. I have several pain management offices that I code for.
As far as I know, all insurance require all patients that undergo spinal cord stimulator trials or intrathecal pain pump trials must at least complete the Psych questionaire (P3) or be seen by credentialed professional for a Psych evaluation.
