Scenario:
In our adult psychiatric hospital, we have patients that are now stable on their medications; however, if they were discharged into the community (shelter because they are homeless or have nowhere else to go), they would not receive the treatment or medications required. This could pose harm to themselves or others. They need step-down level of care (group home, residential treatment facility), but there is no space available or have been rejected. These patients remain in our facility for an undetermined amount of time. We are not a long-term care facility.

Question:
Can we bill a low level 99231 for our daily rounds? There is a stable chronic condition and medication management. The medical necessity being that there is no other place for them to go to receive care. With clear documentation, does this meet medical necessity for billing?
