My providers document behavior as an element of the psychiatric exam. I don't see it on the CMS 1997 guideline chart and I can't see where to count it. Generally it's documented something like this: Cooperative, makes eye contact. Any thoughts?

Personally I think that eye contact fits in "Attention Span and Concentration."

As for cooperative, I don't see why it can't go under Mood and Affect.
