I am a new coder in an Internal Medicine office with one provider. I had a Medicare claim reduced for a 99215 office visit with the remark "Psychiatric Reduction." The ICD-9 codes submitted were 307.81(tension HA), 585.6 (kidney failure), V58.61 (long term anticoag use), 403.11 (hypertensive kidney disease), 285.9 (anemia) and 530.81 (GERD). Any feedback someone could give as to why this claim had a "Psychiatric Reduction" would be greatly appreciated. As far as I could tell, there were no recent IP hospitalizations for Psych reason. Thanks!
