I understand the charges for Psychotherapy are based on the time spent during the encounter. 16-37 mins for 90832, 38-52 mins for 90834 and 53+mins for 90837. My question is that if the time is not documented specifically, is it possible to bill for the lowest code, 90832, as a default? Any guidance would be appreciated.
 
No I don’t think you can default to the lowest since anything less than 16 minutes is not a billable service. So if time is not documented it will be considered as less than 16 minutes
