Would it be logical that the expected outcome of psychotherapy refer to a treatment plan that is many years old? I would think that it would be periodically reviewed.

So my question is - a.) can it be referred to from a previous date and b.) can it be many years old?

I would think its ok to reference so long as you give exact date of what to reference. I would however expect to see perodic updates to it regarding the goals and where they are at. I would be very concerned if there was no updates or anything. I could be wrong as I am not an MD, but I would think if you have a plan and it is not going anywhere, you would need to alter it or indicate updates, etc
 
Not sure where you are, but most payers require a periodic update of the treatment plan. Medicaid in our state requires an updated treatment plan every 90 days, which usually covers any requirement other payers have. You might check to see if the payer has some sort of time line.
