I have a patient who stepped on a nail. The provider anesthetized & cleansed the puncture wound, he opened wound up wider with a #15 blade to due further exploration through the dermal layer

I'm not sure whether to bill an OV level or some other wound CPT code.
 
Take a look at 20103 to see if it applies.
