We just started a new office and I don't know where to apply for a PTAN #.
Can anyone help me with this? Thanks.
The Dr. worked for a Hospital at his last location.
 
The PTAN is the term Medicare uses for the Doctor's Medicare provider number. You would need to contract with Medicare in order to obtain a PTAN. Hope this helps.
