Hope you all can help me with this note's Dx codes. Here goes:

He has an extensive cardiac history including CAD w/PTCA 5 years ago and CABG 2 years ago; HTN, MI 3 years ago, CHF, and A-Fib. He is currently on Lasix, Digoxin, Coumadin, Hydrocholorothiazide, and Nitroglcycerin. Work up revealed, Dx: Acute CHF.

Also, how would you sequence the Dx codes?

It would depend on why he saw the MD and which dx the MD actually treated. If CHF and the rest are history, 428.0, 412, v45.81, v45.82. If he actually changed meds. ordered labs for other dxs - 428.0, 401.9, 427.31, 412, V45.81, v45.82.

Without seeing the A/P, that's as close as I can get, sorry.
