How would you bill Medicare for pterygium excision x2 on the same eye? One is on the nasal side. The other is on the temporal side. Can this be billed as 65420 with 2 units? Or would you bill 65420, and 65420-59?

I would code them on 2 sep lines, with the 59 mod on the second line... while the code description does state that the Pteryguim originates on the nasal side, there are cases where it can form on the temporal side as well...
