Wiki Pulling Smoker Status from Social History?

I realize we typically do not code from Social History, but I am wondering if it is kosher when the provider lists HTN or another heart/cancer related dx so check the smoking status from the social history and code from that history? My ecoder is continuously asking if the pt is a smoker or has a hx of smoking, and it seems unrealistic for the provider to have to state this each time they see a patient.

Please help! My team is up in the air on this one, and no one has found an answer to this question.
