Diagnostic Findings
Severely dilated RV making catheter and wire manipulation and placement in PA/segmental vessels challenging.
Significant thrombus noted in distal right PA with very minimal flow in all segments. No appreciable flow noted in lower segments.
Thrombus noted in left distal PA with diminished flow in all segments; left in left side is better compared to the right side.
Embolectomy performed in the lower right segment, middle segment and upper segment vessels using penumbra lightening 16.
Embolectomy performed in left lower segment vessel which has significant diminished flow using penumbra lightening 16.
Post embolectomy, improved flow noted in right-sided segmental and subsegmental vessels especially in lower segments.
Post embolectomy improved flow noted in left lower lobe segmental vessels
Mean PA pressure improved by 5-8 mmHg pre and post embolectomy in the main PA
Conclusions
Successful pulmonary embolectomy of distal right PA embolus left lower segmental vessel embolus.
Improvement in PA pressures noted in pre and post embolectomy.
Think I'm way over thinking this one. Any help is appreciated.
