When billing CPT's 94200, 94761, 94620, 94010 in a UB04 for outpatient theapy performed in a clinic, we need the revenue codes, ocurrence codes, modifiers and values codes. We tried using revenue code 410, Value code 50 and ocurrence code 11 ,29 &35 and modifier GP. Any help will be appreciated :(
