We just recently told our provider that the CPT code for PRF in the intercostal nerve area should be billed with code 64999. He was wondering since local anesthetic is injected during PRF, can we bill out CPT code 64421 as an alternative? (We told him Medicare won't cover 64999, so we need him to start having pt's sign an ABN.)

I think that is undercoding though, b/c we would only be reporting part of the procedure being done.

Any suggestions?
