I am having problems with BCBS out of state paying for pump refills and medicine. We are attaching the notes and the invoice. However I do not know how to convert the medicine so that they understand it. Does anyone have a template or some advise. thanks

What is the reason for the denial? Have you called to talk with someone in their medical management department to see if there is something specific they are looking for in the claim?
