Hello there, So the company I work for tells us to always put a Q modifier. but the problem is half the time I only have enough information for 1 class B finding and 1 class C finding. So what the heck do I do? Do I just not put a Q modifier? I have searched the internet and I cannot find any information other than there HAS to be a Q modifier or the claim will be denied.
 
We have the same problem here. You just leave the Q modifier off and let your providers/office manager know that you do not have sufficient documentation for billing
