Is Q0091 only used for Medicare and Medicare Replacements? Also, can you only bill this code of the cervix is visualized? What is the difference between Q0091 and G0101? Would G0101 be for Medicare and Medicare replacements as well but 99459 for commercial? Thanks so much!

Replacements of what? Q0091 represents the collection of the pap and handling of a pap smear specimen for a Medicare and sometimes a Medicaid patient (each Medicaid program can make up its own rules on billing this code). If there was no specimen collected you would not bill this code. G0101 is the provider work involved in the examination of 7 of the 11 listed elements. 99459 is a code that covers the cost of using a chaperone and the pelvic pack. Medicare already includes the pelvic pack in the relative value units for G0101 so although they are silent on paying on it, is is an "active" code in their system. Commercial payers generally do not reimburse G0101 or Q0091.
