When a locum tenen sees the pt in the hospital as an IP and then does a procedure on that pt, what is the order for the modifiers. Q6 then 25 or 25 then Q6?

I would use 25 first. Pricing and payment modifiers come before informational modifiers.

I agree. While there are no correct coding rules for the order of modifiers, some payer systems may fail to recognize modifiers that fall further down in the sequence, so it is a best practice to sequence modifiers based on a priority to those that can affect the payment or prevent a denial.
